Vehicle Repair Authorization

I acknowledge that Optimum Collision Lawton may collect and store personal and vehicle information necessary to complete the repair process.

This information may include:

• Name
• Contact information
• Vehicle details
• Insurance claim information
• Photographs of vehicle damage
• Payment information when applicable

I understand that this information may be used for preparing estimates, communicating about repairs, coordinating with insurance companies, ordering parts, completing repairs, processing payments, and maintaining repair records.

I understand that necessary information may be shared with insurance companies, parts suppliers, repair management systems, or law enforcement when required by law.

Optimum Collision Lawton does not sell or distribute customer information to third parties for marketing purposes.

I understand that Optimum Collision Lawton has limited vehicle storage capacity and that vehicles left at the facility may incur storage charges as permitted under Oklahoma law.

Storage fees may apply to vehicles that:

• Are determined to be a total loss
• Are placed on hold awaiting insurance approval beyond seventy-two (72) hours after initial contact

Storage rates may include:

• Up to $39.00 per day for the first ten (10) days
• Up to $75.00 per day beginning on the eleventh (11th) day

Vehicles with damaged lithium-ion batteries requiring specialized storage may incur higher rates due to manufacturer safety requirements:

• Up to $125.00 per day for the first ten (10) days
• Up to $200.00 per day beginning on the eleventh (11th) day

I authorize Optimum Collision Lawton to communicate with my insurance company regarding the estimate, repair status, and any applicable storage charges.

I understand that all repair costs, administrative fees, and storage charges must be paid or otherwise resolved before the vehicle will be released.

Optimum Collision Lawton is not responsible for loss or damage to vehicles, articles, or personal property left in vehicles, in case of fire, theft, natural disasters or any other cause beyond our control. Please remove all personal belongings from your vehicle.
No car will be released until full payment is made. Deductible and insurance check or proof of loss must be received before vehicle will be released. Be certain all payee's (including lien holders) have endorsed the insurance check prior to completion of repairs.

Vehicles that have sustained extensive damage could require more time to repair and exceed your rental coverage. Optimum Collision Lawton will not be responsible for any rental expense in the event this occurs.

Customer understands that this vehicle is subject to repossession in accordance with the relevant provisions of the Oklahoma Uniform Commercial Code (UCC) and any other applicable Oklahoma statutes, if a written order of payment for repairs of the vehicle is stopped, or dishonored because of insufficient funds, no funds, or because the drawer or maker of the order has no account or the account on which it is drawn has been closed.

I understand that modern vehicles are complex and that electronic data recorded in vehicle systems may help to diagnose necessary repairs and to confirm that applicable systems are functional after repairs are performed. I authorize Optimum Collision Lawton to access this data as part of the diagnostic and repair process.

I authorize Optimum Collision Lawton to complete estimated repair work to be done along with the necessary materials. And hereby grant you, and/or your employees' permission to operate the vehicle herein described on streets, highways or elsewhere for the purpose of completing repairs, testing and/or inspection. An express mechanic's lien is hereby acknowledged on above vehicle to secure the amount of repairs thereto.

I understand that signing the following document I give Optimum Collision Lawton temporary Power of Attorney effective for 180 days from sign date to; Endorse insurance checks on my behalf in relation to services rendered on my vehicle.
